This website collects nothing

There are no forms on this site, no account to create, no newsletter, no analytics and no advertising trackers. You can read every page without telling us who you are. If you want to reach us you either call a clinic or send an email, and in both cases you decide what to share.

If you email or call us

We keep what you send us, for as long as we need it to deal with your question, and we do not sell it or pass it to anyone marketing anything. Email is not a secure channel, so please do not send medical details or anything sensitive by email. Call the clinic instead.

Your medical records are a separate matter

Records created during a disability evaluation are handled under HIPAA, not under this page. Patriot Care Center complies with the HIPAA standards for privacy, electronic transactions and security, including the HITECH Act and the Omnibus Rule of 2013. We maintain written policies and procedures for federal security requirements, monitor compliance on an ongoing basis, and keep the reporting required of us by law.

The report from an evaluation belongs to the file it was requested for, so it goes back to whoever asked for the exam. If you want a copy, the Department of Veterans Affairs can tell you how to request one.
